Camper Registration Form
Camp starts July 8th and ends Aug 16th, 2024
10:15 a.m.-3:15 p.m.
Complete the online form below and then submit. Be sure to read the details outlined in the consent form and check the box “YES” at the end of the registration form. Then submit this form. The camper registration and consent form will be processed together.

I agree that should my child bring items of a personal nature to Camp Dakota, the employees/counsellors shall not be held responsible for loss or damage to any item.
I give consent for my child’s photograph to be used in pamphlets, information brochures and on websites for the purpose of fundraising and to promote Camp Dakota/Front Line Outreach non-profit programs and events. (children’s name will not be used.)
I give consent for Camp Dakota/Front Line Outreach to share my child’s’ information with Jump Start Canada for the sole purpose of receiving funding to further supply Camp Dakota with materials necessary to support sports, games and physical activity during camp.
I agree not to hold Camp Dakota, Front Line Outreach or their employees responsible for an accident, illness or injury incurred by my child while participating in camp activities or to hold them responsible for any circumstances that may occur should my child leave camp premises without counsellor accompaniment.
I understand that my child will not be allowed to be picked up by any person not listed on this registration except with prior permission from the parent/ guardian listed on this form.
I give consent for Camp Dakota to photograph my child and keep this photo on file for the duration of camp in the event that my child leaves the camp area without a counsellor and a current photo is needed if my child is not located immediately.
